PLH Connect
PLH Connect supports young people, their families, and support networks who are navigating complex challenges by bringing local services and collaborative supports together around their wellbeing.
Using a flexible, relationship-based and place-based approach, the program works alongside young people in the environments where they feel most comfortable, strengthening connection, engagement, confidence, and pathways toward positive futures. PLH Connect is proudly based in Roma and funded by the Western Queensland Primary Health Network (WQPHN).

We help families find, understand and stay connected with the right health, education and community services.


Working alongside HYPE and lcoal providers, the program offers trauma-informed, non-judgemental support and builds strong relationships with families to reduce crises and strengthen long-term outcomes.
