PLH Connect
PLH Connect is a Peace Lily House progam, funded by the Western Queensland Primary Health Network. It supports individuals and families experiencing complex challenges find local services to support collaborate in support of their wellbeing.
We help families find, understand and stay connected with the right health, education and community services.
Working alongside HYPE and lcoal providers, the program offers trauma-informed, non-judgemental support and builds strong relationships with families to reduce crises and strengthen long-term outcomes.
